Elements and Performance Criteria

1. Elements define the essential outcomes.
2. Plan a health program as part of primary health care services
   • Identify community health needs in partnership with the community
   • Review, with primary health team, existing health program activities for effectiveness in addressing identified community health needs
   • Establish priorities for development of health programs
   • Identify relevant community members, primary health staff and other health professionals to support the development of health programs
   • Identify necessary resources and availability
3. Develop a health program as part of primary health care services
   • Design program in partnership with key representatives of the community and relevant agencies/organisations
   • Identify appropriate delivery strategies according to the needs of the target group, location and health issue
   • Promote client self-care approach to health as part of health program
   • Ensure health program has a holistic, culturally sensitive approach
   • Develop effective measures to ensure record maintenance, confidentiality and compliance with legislation
   • Design evaluation criteria and processes to measure the program’s effectiveness in addressing identified community health objectives
4. Evaluate health program as part of a primary health care service
   • Review program records to evaluate program against criteria that reflect identified community health objectives
   • Identify program strengths and areas for improvement as a basis for continual enhancement of health outcomes
   • Seek feedback on program effectiveness and provide to community and organisation representatives
   • Communicate outcomes of the evaluation to relevant stakeholders to guide future activities in the delivery of health services to the community

Performance Evidence

The candidate must show evidence of the ability to complete tasks outlined in elements and performance criteria of this unit, manage tasks and manage contingencies in the context of the job role.

There must be evidence that the candidate has:

planned and developed at least two programs as part of primary health care services across a range of contexts, including:

assessing and addressing community health needs

consulting key community representatives and related agencies/organisations

analysing funding requirements for programs to prepare proposal/submission

identifying appropriate delivery and evaluation strategies

accessing, adopting or developing required resources

evaluated at least one health program as part of primary health care services by:

assessing program against criteria that reflects identified community health objectives

identifying program strengths and areas for improvement

seeking feedback on program effectiveness and providing feedback to community and organisations.

Knowledge Evidence

The candidate must be able to demonstrate essential knowledge required to effectively do the task outlined in elements and performance criteria of this unit, manage the task and manage contingencies in the context of the identified work role.

This includes knowledge of:

relevant policies, protocols and procedures of the organisation

health programs relevant to the area/region/community

management of common existing and potential health problems for the community

effective strategies to monitor and manage identified health problems in line with community needs and sensitivities

occupational health and safety practices and procedures relevant to the work (e.g. manual handling)

available health and support services in the region.
